COME WORK WITH US! Great opportunity for someone that has a passion to work with Intellectually Disabled clients in the Santa Clara area providing Supportive Living Services. We are looking for a part-time Case Manager to start (If looking for full time you can fill in for DCS shifts until the position becomes Full-Time) Weekends are a must. There is an on call component. 22$ an hour to start.

Education: Degree is great but not required. We are looking for experience.
Experience: People Management experience
Experience in case management working for companies providing services for disabled individuals.
Experience working with Mental Health/Intellectual Disabilities or

General information of position:
- Providing supervision of Direct Care staff and coverage coordination.
- Training and supervision of staff.
- Attentiveness of reportable events and submitting reports within required time frames.
- Manages and monitor client finances within required time frames.
- Serve as liaison with service coordinators, families, other agencies, and internal staff.
- Manages client caseloads, weekly review of client charts/records, and monitoring daily shift notes.
- Manages the supervision and evaluation of service delivery quality and content program to clients.
- Coordinates and ensures staff and the program’s daily activities are completed efficiently.
- Must maintain current working knowledge of Supported Living Services, Federal & State laws, including familiarity with Title 17 regulations.
- Maintains a prominent level of confidentiality.
- Support the goals in creating a super work environment.
